Sometimes the problem isn’t another medication. It may be the medications they are already taking.
Older adults often receive care from multiple providers.
A primary care provider may prescribe one medication. A cardiologist may prescribe another. A neurologist may add another. A hospital may add two more. Then an over-the-counter medication or supplement is added at home.
Before you know it, the medication list has grown. And sometimes no one has had the opportunity to sit down and look at everything together.
☐ Does my loved one take five or more medications?
☐ Do they see several doctors or specialists?
☐ Have they recently been hospitalized or visited the emergency department?
☐ Has anyone recently complained of dizziness, weakness, confusion or excessive sleepiness?
☐ Have they had a recent fall?
☐ Are they taking medications that were prescribed months or years ago?
☐ Do they use over-the-counter medications or supplements?
☐ Are there medications in the home that no one is sure they should still be taking?
☐ Has a doctor told them to stop a medication, but it is still on the medication list or still being taken?
☐ Are you the caregiver, and are you unsure whether your loved one’s medication list is accurate?
If you answered YES to any of these questions, a medication optimization review may be appropriate.

Geriatric Medication Optimization & Safety Review
Available virtually or in the comfort of your loved one’s home.
One patient. One complete medication picture. One coordinated plan.
As we age, medications can quickly accumulate. Prescriptions from primary care providers and specialists, hospital discharge medications, over-the-counter products, vitamins, and supplements can create a complicated medication regimen.
Our Geriatric Medication Optimization & Safety Review helps older adults and their families understand the complete medication picture and identify potential safety concerns.
This service may be especially helpful for older adults who:
A comprehensive look at the entire medication regimen
Prescription medications
Medications from primary care, specialists, urgent care, and hospitals.
Over-the-counter medications & supplements
Products that may not appear on the medical record but can still affect safety.
Medication reconciliation
Comparison of the patient’s medication list with what they are actually taking.
Geriatric medication safety
Review using evidence-based geriatric prescribing principles, including the American Geriatrics Society Beers Criteria® when appropriate.
Potential medication risks
Assessment for potential duplication, drug-drug interactions, drug-disease concerns, sedation, cognitive effects, fall risk, and other clinically relevant issues.
Medication simplification & deprescribing opportunities
Identification of medications that may warrant discussion with the prescribing clinician regarding dose adjustment, discontinuation, or simplification when clinically appropriate.
Greater clarity
Know what medications are being taken, why they are being taken, and how they fit together.
Improved medication safety
Identify potential medication-related risks before they become bigger problems.
Better coordination
Help caregivers and healthcare providers have a clearer, more complete medication picture.
Peace of mind for caregivers
Get professional guidance when managing a loved one’s medications becomes overwhelming.
A geriatric-focused perspective
Older adults can respond differently to medications. Our review considers age-related safety concerns and the patient’s individual circumstances.
VIRTUAL TELEHEALTH REVIEW
Convenient medication review from the comfort of home.
IN-HOME MEDICATION REVIEW
For eligible patients, we can review medication bottles, OTC products, supplements, and medication routines in the home.
Every medication should have a purpose, a reason, and a place in the patient’s overall plan of care.
We do not simply count medications. We look at the whole picture and identify opportunities to improve medication safety, reduce unnecessary complexity, and support informed conversations with the patient’s healthcare team.

The AGS Beers Criteria® is one component of a comprehensive medication review and does not automatically mean a medication should be stopped. Medication changes should be made only under the direction of an appropriate prescribing clinician.
This service may be especially helpful for older adults who take multiple prescription medications, see several physicians or specialists, have recently been hospitalized or visited the emergency department, have experienced falls, dizziness, weakness, excessive sleepiness, or confusion, use over-the-counter products or supplements, have frequent medication changes, or have a caregiver who is unsure what medications are actually being taken.
The review may include prescription medications, medications from primary care, specialists, urgent care and hospitals, over-the-counter medications, vitamins, supplements, PRN medications, medication doses and schedules, and what the patient is actually taking at home.
The review may assess potential duplication, drug-drug interactions, drug-disease concerns, sedation, cognitive effects, fall risk, age-related safety issues, and other clinically relevant concerns.
Medication reconciliation compares the patient's medication list with what they are actually taking. This helps create a clearer and more complete medication picture.
The review may identify medications that warrant discussion with the prescribing clinician regarding dose adjustment, discontinuation, or simplification when clinically appropriate. Medication changes should only be made under the direction of an appropriate prescribing clinician.
